LIBRARY CARDS FOR PROVO CITY EDUCATORS

Provo City Library waives the nonresident fee for educators working in the Provo City School District.

HOW TO OBTAIN A PROVO CITY EDUCATOR LIBRARY CARD:

  • Complete an application available at the Circulation Help Desk or print a card application
  • Present the completed application along with ALL of the following to the Circulation Help Desk:
    • Address Verification - Mail postmarked with the last 3 months, a rent receipt/contract, or preprinted checks
    • Photo Identification - Picture ID issued by a government, school, or employer
    • Proof of Employment at a Provo School - School Picture ID, insurance card, or pay stub from the school

  • Use the educator library card for materials to supplement your teaching.
  • Library materials are for classroom use only, not to be sent home with students.
  • You are financially responsible for all items on your card.
  • Please refrain from checking out all items on a particular topic.


TARJETAS DE LA BIBLIOTECA POR EDUCADORES DE LA CIUDAD DE PROVO

La biblioteca de la ciudad de Provo renuncia a la tarifa de no residente por educadores que trabajan para el Distrito Escolar de la Ciudad de Provo.

CÓMO OBTENER UNA TARJETA DE BIBLIOTECA DE EDUCADOR DE LA CIUDAD DE PROVO:

  • Completar una solicitud en el Circulation Help Desk o imprime La Solicitud de Tarjeta de Provo City Library 
  • Presente la solicitud completa junto con TODO lo siguiente al Circulation Help Desk:
    • Verificación de domicilio: correo con matasellos de los últimos 3 meses, un recibo/contrato de alquiler o cheques preimpresos
    • Identificación con foto: Identificación con foto emitida por un gobierno, escuela o empleador
    • Prueba de empleo en una escuela de Provo: Identificación con foto de la escuela, tarjeta de seguro o talón de pago de la escuela

Recursos para Educadores

PAUTAS PARA EL EDUCADOR:

  • Use la tarjeta de la biblioteca del educador para obtener materiales que complementen su enseñanza.
  • Usa materiales en su salón de clases. Los materiales de la biblioteca no deben enviarse a casa con los estudiantes.
  • Usted es financieramente responsable de todos los materiales en su cuenta.
  • Favor de absténer de tomar prestado todos los artículos de un tema en particular.
